How to apply for a medical card:

1. Stop into The Green Bean and fill out paperwork for the doctor. Please bring a valid form of ID with you.
2. Pay the fee of $75 for renewals and $100 for new patients
3. We will schedule you an appointment on a Sunday afternoon
4. If you are a new patient you will need medical records by the time of your appointment.

Arriving at your appointment:

1. Call 810-232-4400 and let The Green Bean know you are here for your appointment.
2. Hang tight in your vehicle until we call you back for your appt, unless we tell you otherwise.
3. After your appointment, we will give you your documentation to send to the state.
